Client Forms

Please follow each of the links below to fill out the forms. All 5 forms NEED to be completed before your first appointment.

Client Information – we require your contact information, your OHIP information (if applicable) and your doctor’s contact information. Signature required.

Request for Records – this document needs to be signed so that we are able to request copies of any pregnancy related reports that would be needed for your care.

Consent & Release for Email & Text Communication – it is very important that you thoroughly read through this document and sign it with your consenting decision. The form includes a section to agree/decline email and texting communication.

Anti Harrassment Agreement

THP Pre-Registration form – this form needs to be submitted to register yourself at the hospital for your delivery. We will submit the form on your behalf once completed.

OPTIONAL FORM

Pregnancy Check-In – a questionnaire that your midwives will talk to you about throughout your pregnancy. Ideally you will complete this form before your first appointment and your midwives will use it to talk to you about throughout your pregnancy.
